APPLICANT REQUESTS THAT:

1.  Her records be updated to reflect air force specialty code 
(AFSC) 3S051, Personnel Journeyman, instead of 3S031, Personnel 
Apprentice.  

2.  Her records be corrected to reflect the last name of instead 
of .

APPLICANT CONTENDS THAT:

Her records were not updated to reflect the five skill level she 
worked hard to obtain.  Also, she changed her name back to and 
would like for her records to be annotated as such.  She would 
like these corrections made so that she may rejoin the military.  

The applicant’s complete submission, with attachments, is at 
Exhibit A.

STATEMENT OF FACTS:

The applicant’s military personnel records indicate she enlisted 
in the Regular Air Force on 6 Mar 07.

On 20 Feb 09, the applicant was furnished a general (under 
honorable conditions) discharge with a narrative reason for 
separation of a pattern of misconduct.  She was credited with 
1 year, 11 months, and 15 days of total active service.  The 
applicant’s records do not contain any documents related to the 
applicant’s general discharge, so the circumstances surrounding 
her discharge could not be verified.  However, the presumption 
of regularity dictates that, absent evidence to the contrary, 
that the discharge was carried out in accordance with the 
governing instructions and was within the discretion of the 
discharge authority.  
The remaining relevant facts pertaining to this application are 
described in the letters prepared by the Air Force offices of 
primary responsibility, which are attached at Exhibits C and D.

AIR FORCE EVALUATION:

AFPC/DPSIC recommends approval of the applicant’s request to 
have her skill level updated to reflect 3S051.  According to the 
applicant’s AF Form 910, Enlisted Performance Report (AB thru 
TSgt), during the period 6 Mar 07 thru 5 Nov 08, her duty AFSC 
is listed at 3S051, Personnel Journeyman.  Since she performed 
as a 3S051, it should be annotated in her records.  

A complete copy of the AFPC/DPSIC evaluation is at Exhibit C.  

AFPC/DPSIRP recommends denial of the applicant’s request for her 
records to reflect a last name change.  In accordance with Air 
Force Instruction, 36-2608, Military Personnel Records System, 
Table A7.2, the applicant must provide the appropriate legal 
documentation to support a name change.  In this case, a review 
of the applicant’s official military personnel file (OMPF) shows 
her official last name at the time of her discharge is the same 
name as reflected on her DD Form 214.  Since the applicant did 
not submit an original or certified copy of the required legal 
documentation to support her request for a name change and she 
has no continuing affiliation with the Air Force as a Reserve 
member or retiree, there is no basis to amend her military 
records.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION:

Copies of the Air Force evaluation were forwarded to the 
applicant on 24 Feb 14 for review and comment within 30 days.  
As of this date, no response has been received by this office 
(Exhibit E).

THE BOARD CONCLUDES THAT:

1.  The applicant has exhausted all remedies provided by 
existing law or regulations.

2.  The application was not timely filed; however, it is in the 
interest of justice to excuse the failure to timely file.

3.  Insufficient relevant evidence has been presented to 
demonstrate the existence of an error or injustice with respect 
to the applicant’s request to change her last name as it appears 
in her military records.  We took notice of the applicant’s 
complete submission in judging the merits of the case; however, 
we agree with the opinion and recommendation of AFPC/DPSIRP and 
adopt its rationale as the basis for our conclusion the 
applicant has not been the victim of an error or injustice with 
respect to her request for a name change.  

4.  Notwithstanding the above, sufficient relevant evidence has 
been presented to demonstrate the existence of an error or 
injustice in regards to the applicant’s request for correction 
of her duty air force specialty code (DAFSC), as it appears on 
her DD Form 214.  We took notice of the applicant’s complete 
submission in judging the merits of the case and agree with the 
opinion and recommendation of AFPC/DPSIC and adopt its rationale 
as the basis for our conclusion that corrective action is 
warranted.  Therefore, we recommend the applicant’s record be 
corrected to the extent indicated below.

THE BOARD DETERMINES THAT:

The pertinent military records of the Department of the Air 
Force relating to the APPLICANT be corrected to show that her DD 
Form 214, Certificate of Release or Discharge from Active Duty, 
issued in conjunction with her 20 February 2009 discharge, be 
corrected in Block 11, Primary Specialty, to reflect “3S051, 
Personnel Journeyman,” rather than “3S031, Personnel 
Apprentice”.
